New Injury Evaluations
Athletes in grades 9-12 with a new injury should report to the Lovejoy High School Athletic Complex - Athletic Training Room. Athletes in 7- 8th grade will report to the WSMS athletic training room in the auxiliary gym. Athletes should report for evaluations at 7:30 a.m. on school days.

Physical and Medical History
Physical/Med Hx forms may be picked up from the LHS, WSMS or LISD administration front offices. Physical forms can be printed from UIL Physical form to 2021-22
Where Physical/Medical Hx forms can be returned
1. Physical/Medical Hx forms can be sent in electronically via email. Please scan into a PDF format and double check the borders so we can see the entire form.
Please send boys' forms to andrew_smith@lovejoyisd.net
Please send girls' forms to sarah_seedle@lovejoyisd.net
2. Dropped off at the front office of LHS, WSMS or LISD Administration
